like do you mean, it says there are 2 comments, but you go to click on it and it says zero and puts you into a comment text box, or the next comment? -
If you're talking about when using the Questions section below video and article content, then I have an answer that I've deduced myself, so it may be inaccurate. I think it's when a user comments on a question, and then deletes it later. When a comment is deleted, it stays on Khan Academy servers, I'm guessing so that guardians can view them still. I know this because if you have a link to a specific comment that has since been deleted, it will display the comment, but the avatar will have been removed, and the box is outlined with red, accompanied by a bold, red "DELETED" The Khan Academy system will see that there are deleted comments and will still count them as there, but when the system goes to display the deleted comments, it will go "Oh, oops, these are deleted" and display nothing. Take this, as Sal would say, "with a grain of salt" because this is not backed by any KA guardians.

I mean, if there were six comments, and then I click on those six comments, I don’t see six comments, and then I see two comments. Or maybe I click on those six comments, and then there are no comments. Get it? And I get why that happens.

This happens to me frequently. So many weird things happen on my account that KA support staff claim they can't duplicate that I have given up reporting them.
If you are able to make a video showing it happening, I suggest recording such a video and sharing it with KA support staff through the "Report a Problem" link in the upper right. I think there are interface problems.

Sometimes commenting in the Support Community helps; it varies widely.
