Student scores shows 100 but no attempts

I have a few students that have gotten 100 on an exercise but when I check on the report it shows zero attempts and no student answers.  Another student it shows that they only answered 1 of the 4 questions, yet their score is 100.  I'm wodering if it is just server/internet issues or if some students found some work around.
