If you've filled out the form previously, then you should have received a message in your notifications explaining the duration of the ban and the reason for it. The message would have been sent either when your account was banned or when it was unbanned.

If you can't find the notification, you can resubmit the form requesting that information about the ban be sent to you.

See, here's the thing: I'm on a child account (Mac), and I had to have the ban-appeal approved by my 'rents, so I filled it then. Now I've switched computers, and thus, accounts.
